Probiotics vs. Prebiotics: What’s the Difference, and Do You Actually Need Them?

If you've searched for ways to manage irritable bowel syndrome (IBS), you've probably come across probiotics. They're marketed for bloating, digestion, gut health, and nearly everything in between.
But do probiotics actually help IBS?
The short answer is: sometimes—but it depends on the probiotic.
Not all probiotics work the same way. In fact, one of the biggest misconceptions is that any probiotic will improve IBS symptoms. Current research tells us that's simply not true.
Here's what the evidence says, who might benefit, and how to decide whether probiotics are worth trying.

One reason the research seems confusing is that studies often evaluate completely different probiotic strains, doses, and combinations. Pooling them together makes it difficult to know which products truly work.

Foods like yogurt, kefir, kimchi, and sauerkraut contain beneficial microbes and can be nutritious additions to many eating patterns.
They may support overall gut health, but we don't yet have strong evidence that eating these foods alone consistently improves IBS symptoms.
That's because the bacteria in foods vary widely and usually don't contain the specific strains studied in IBS research.

That doesn't mean one is "better" than the other. They simply serve different purposes.
Fermented foods can absolutely fit into a balanced diet if you enjoy them and tolerate them well.

Walking down the supplement aisle can be overwhelming. There are hundreds of probiotic products, but most have never been studied for IBS.
Here are five things to look for before buying a probiotic.

Probiotics are strain-specific, meaning a probiotic that helps one condition may not help another—even if it's the same species of bacteria.

Some recent research suggests Bacillus coagulans may have one of the highest probabilities of improving overall IBS symptoms, abdominal pain, bloating, and straining. While these findings are promising, no single probiotic works for everyone, and more research is still needed.
Research suggests most probiotics studied for IBS provide at least 1 billion CFU (10⁹ CFU) per day, and improvements typically aren't seen overnight.

When shopping for a probiotic, check for:
✔️ The full strain name, not just the species (for example, Lactiplantibacillus plantarum 299v, not simply "Lactobacillus")
✔️ A dose of at least 1 billion CFU per day
✔️ CFU guaranteed through the expiration date, rather than only "at time of manufacture"
✔️ Storage instructions that match the product (some require refrigeration, while others are shelf stable)
✔️ Independent quality testing, such as USP, NSF, or ConsumerLab certification, when available
These details provide greater confidence that you're purchasing a product similar to the ones studied in clinical trials.
A probiotic containing 20 or 30 different strains isn't automatically better.
In fact, adding more strains doesn't necessarily improve results. Some multi-strain products have been helpful in studies, while others have shown no benefit—and at least one combination has even performed worse than placebo.
Instead of choosing the product with the longest ingredient list, focus on whether the specific strain has been studied for IBS.
If you have IBS, probiotics may help—but choosing one isn't as simple as grabbing the first bottle on the shelf.
